How can I productively react when students misbehave and act up?

The best way to react when students misbehave is to already have the steps mapped out. This will be stated already because I will go over it at the very beginning of the school year. These consequences will be known by the student, therefore this should make the process smoother. I how to do this through our text book and the discussion we had about it in class. I know it is important to have the consequences already mapped out, but what are appropriate consequences?
